Use this form to register as a borrower and obtain a PIN to request articles or books that are not held at PMH Library. Complete the form and either fax (9340 7441) or send to the PMH Medical Library. See Services for information about borrowing from the library.

Request table of contents alerts (eTOCS)
To receive the latest table of contents from your favourite journals, use this form to sign up for the eTOCS service. Complete the form and either fax (9340 7441) or send to the PMH Medical Library. Or the library.
